Treatment of acid reflux (GERD), heartburn, erosive esophagitis; peptic/duodenal ulcers; gastritis and acid-related indigestion; prevention/treatment of NSAID-induced ulcers; part of H. pylori eradication therapy (with antibiotics); Zollinger–Ellison syndrome

Pantoprazole is a proton pump inhibitor (PPI). It irreversibly blocks the gastric H+/K+ ATPase (proton pump) in stomach parietal cells, reducing acid secretion and allowing ulcers/erosions to heal.

Take as prescribed, usually 1 tablet once daily. Swallow whole (do not crush/chew) preferably 30–60 minutes before food, ideally before breakfast. Duration depends on condition; if used with antibiotics for H. pylori, follow the full regimen.

Headache, diarrhea, nausea, abdominal pain, flatulence, vomiting, dizziness, constipation

Use with caution in severe liver disease. Long-term use may increase risk of vitamin B12 deficiency, low magnesium, bone fractures, and infections (e.g., C. difficile). Seek medical advice for alarm symptoms (unexplained weight loss, persistent vomiting, black stools, blood in vomit, difficulty swallowing). May mask symptoms of gastric cancer—evaluate if persistent/recurrent. If kidney problems occur (reduced urine, swelling), stop and consult doctor (rare interstitial nephritis).

Warfarin (monitor INR), methotrexate (high-dose; increased levels), digoxin (risk increased with low magnesium), ketoconazole/itraconazole/posaconazole (reduced absorption), erlotinib (reduced effect), atazanavir/nelfinavir (reduced antiviral levels; avoid), clopidogrel (possible reduced activation—clinical significance less than omeprazole but use caution), rifampicin and St John’s wort (may reduce pantoprazole effect)

Store below 25°C in a dry place. Protect from moisture and direct light. Keep in original blister pack and out of reach of children.
