Pioglitazone is a thiazolidinedione that activates PPAR-gamma receptors, improving insulin sensitivity in muscle and adipose tissue and reducing hepatic glucose output. This lowers blood glucose and improves glycaemic control over time.
Take by mouth as advised by the doctor, usually once daily at the same time each day, with or without food. Swallow the tablet with water; continue diet/exercise plan and monitor blood glucose regularly.

Not recommended in type 1 diabetes or diabetic ketoacidosis. Can cause or worsen heart failure—avoid in NYHA class III/IV and use caution in patients with fluid retention; seek help for rapid weight gain, swelling, shortness of breath. Monitor liver function; stop and consult doctor if jaundice or dark urine occurs. May increase risk of fractures (especially in women). Rare risk of macular edema—report vision changes. Use cautiously in elderly and those with heart disease; dose adjustment of concomitant insulin/sulfonylurea may be needed to reduce hypoglycaemia.
Insulin (increased risk of edema/heart failure), sulfonylureas (increased hypoglycaemia risk), gemfibrozil (may increase pioglitazone levels), rifampicin (may reduce pioglitazone effect), oral contraceptives containing ethinyl estradiol/norethindrone (may reduce hormone levels), other hepatotoxic drugs (additive liver risk).
