Monograph
Tablet
Treatment of schizophrenia; treatment of bipolar disorder (manic episodes and bipolar depression); maintenance treatment of bipolar disorder (as advised by psychiatrist); adjunct treatment in major depressive disorder (in selected patients, as prescribed).
Quetiapine is an atypical antipsychotic that modulates neurotransmitters in the brain by antagonizing serotonin (5‑HT2) and dopamine (D2) receptors. Its sustained‑release formulation provides gradual drug release to maintain symptom control with once‑daily dosing.
Oral tablet (sustained release): take exactly as prescribed, usually once daily, preferably in the evening. Swallow whole with water; do not crush, chew, or split. Can be taken with or without food; follow your doctor’s titration schedule and do not stop abruptly.
Sleepiness/drowsiness, dizziness, dry mouth, constipation, increased appetite, weight gain, fatigue, orthostatic hypotension, fast heart rate, blurred vision
Prescription-only antipsychotic. May cause marked drowsiness—avoid driving/alcohol until you know effects. Risk of weight gain, high blood sugar/diabetes and lipid changes—monitor weight, glucose and lipids. Can cause orthostatic hypotension and falls, especially at start or with dose increases. Rare but serious: neuroleptic malignant syndrome, tardive dyskinesia, seizures, severe allergic reactions. QT prolongation risk—use caution in heart disease, low potassium/magnesium, or with QT-prolonging drugs. Increased risk of suicidal thoughts in young adults; monitor mood changes. Elderly patients with dementia-related psychosis: increased risk of death and stroke (not recommended).
CYP3A4 inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) increase levels; CYP3A4 inducers (carbamazepine, phenytoin, rifampicin, St John’s wort) decrease levels. Additive sedation with alcohol, benzodiazepines, opioids, antihistamines. Additive hypotension with antihypertensives. QT-prolonging medicines (amiodarone, sotalol, some macrolides/fluoroquinolones, antipsychotics) increase arrhythmia risk. Levodopa/dopamine agonists may have reduced effect.
