Monograph
Rabeprazole 20 mg + Levosulpiride 75 mg (SR)
Gastroesophageal reflux disease (GERD), acidity/heartburn, erosive gastritis, dyspepsia with nausea/bloating, reflux associated with delayed gastric emptying
Rabeprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase in gastric parietal cells. Levosulpiride is a dopamine D2 antagonist with prokinetic and antiemetic action that improves gastric motility and reduces nausea, helping reflux symptoms.
Tablet/capsule for oral use. Usually taken once daily on an empty stomach, preferably 30 minutes before breakfast (as advised by doctor). Swallow whole; do not crush/chew sustained-release dosage forms.
Headache, dizziness, diarrhea, constipation, abdominal pain, nausea, flatulence, dry mouth, sleepiness, restlessness, increased prolactin (breast tenderness/galactorrhea), rash
Use only on medical advice. Prolonged PPI use may cause low magnesium, vitamin B12 deficiency, increased fracture risk, and risk of C. difficile diarrhea; seek care for persistent diarrhea. Levosulpiride may cause extrapyramidal symptoms (tremor, rigidity), drowsiness, and hyperprolactinemia (menstrual irregularities, breast changes); avoid driving if sleepy. Use with caution in liver/kidney impairment, elderly, history of seizures, Parkinson’s disease, prolactin-dependent tumors, or QT prolongation/arrhythmia. Get evaluated for alarm symptoms (weight loss, GI bleeding, persistent vomiting, difficulty swallowing) before use.
Antacids may reduce absorption if taken together (separate dosing). Drugs needing acidic pH for absorption (e.g., ketoconazole/itraconazole/posaconazole, atazanavir) may be affected by rabeprazole. Warfarin (monitor INR), digoxin (monitor levels), methotrexate (high-dose) interaction possible with PPIs. Levosulpiride: additive CNS depression with alcohol/sedatives, increased risk of QT prolongation with macrolides/fluoroquinolones/antipsychotics, antagonizes dopamine agonists (levodopa), caution with other antipsychotics and serotonergic antiemetics.
