Management of obesity/overweight and weight reduction in adults, as an adjunct to a calorie-restricted, low-fat diet; helps reduce risk factors associated with excess weight (e.g., dyslipidaemia).
Orlistat inhibits gastric and pancreatic lipases in the gut, preventing breakdown and absorption of about 30% of dietary fat. The unabsorbed fat is excreted in stools, leading to reduced calorie intake and weight loss.
Tablet/capsule: Take 120 mg three times daily with each main meal containing fat, during or up to 1 hour after the meal. Skip the dose if a meal is missed or contains no fat. Follow a reduced-calorie, low-fat diet; take a multivitamin (A, D, E, K) at bedtime or at least 2 hours apart.
Oily spotting, flatulence with discharge, urgent bowel movements, increased defecation, fatty/oily stools, abdominal pain/cramps, diarrhoea, headache
Not for use in chronic malabsorption syndrome or cholestasis. Use with caution in patients with history of kidney stones/oxalate nephropathy, liver disease, pancreatitis, hypothyroidism, eating disorders. May reduce absorption of fat-soluble vitamins—consider multivitamin supplementation. Stop and seek medical help if signs of hepatitis (jaundice, dark urine, persistent nausea) or severe abdominal pain occur.
Cyclosporine (reduced levels; separate by at least 3 hours and monitor), warfarin/other anticoagulants (vitamin K reduction may alter INR), levothyroxine (risk of hypothyroidism; separate by at least 4 hours and monitor), antiepileptics such as valproate/lamotrigine (reduced seizure control reported), amiodarone (reduced exposure), antiretrovirals (may reduce absorption), fat-soluble vitamin supplements (separate timing)
