Type 2 diabetes mellitus: to help control blood sugar in adults when diet and exercise alone are not enough, as prescribed.
Metformin decreases glucose production in the liver and improves insulin sensitivity. Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Sitagliptin (a DPP-4 inhibitor) increases incretin levels, improving insulin release and reducing glucagon in a glucose-dependent manner.
Take by mouth exactly as prescribed. Usually taken with food (preferably with/after meals) to reduce stomach upset; swallow whole with water. Do not skip meals; monitor blood glucose regularly.

Risk of hypoglycaemia (higher if meals are missed, with alcohol, or with other antidiabetics); metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, muscle pain, unusual sleepiness. Use caution in kidney or liver disease; dose adjustment/avoidance may be needed. Withhold metformin around iodinated contrast procedures and major surgery as advised. Not for type 1 diabetes or diabetic ketoacidosis. Elderly and malnourished patients are at higher hypoglycaemia risk.
Alcohol, insulin/other antidiabetics (additive hypoglycaemia), corticosteroids (raise glucose), diuretics and sympathomimetics (raise glucose), ACE inhibitors (may enhance glucose-lowering), fluoroquinolones (glycaemic disturbances), beta-blockers (mask hypoglycaemia), iodinated contrast media (metformin—lactic acidosis risk), cationic drugs affecting renal excretion (e.g., cimetidine)
