Type 2 diabetes mellitus (in adults) to improve blood glucose control along with diet and exercise, especially when metformin alone is inadequate
Vildagliptin is a DPP-4 inhibitor that increases incretin levels (GLP-1/GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin reduces hepatic glucose production and improves insulin sensitivity; the SR form provides prolonged action and may improve gastrointestinal tolerability.
Oral tablet. Usually taken once or twice daily as prescribed, preferably with meals to reduce stomach upset. Swallow whole; do not crush or chew sustained-release tablets. Regularly monitor blood glucose and follow diet/exercise advice.
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, upper respiratory tract infection, constipation, flatulence, skin rash
Not for type 1 diabetes or diabetic ketoacidosis. Metformin can rarely cause lactic acidosis—risk increases with kidney impairment, severe infection, dehydration, alcohol use, liver disease, or hypoxia/heart failure; stop and seek care if severe weakness, rapid breathing, abdominal pain, or unusual sleepiness occur. Use with caution/avoid in significant renal impairment; kidney function should be checked before and during therapy. Temporarily withhold metformin before/after iodinated contrast procedures and during major surgery as advised by doctor. Monitor for pancreatitis symptoms (severe persistent abdominal pain) and stop if suspected. Risk of hypoglycaemia increases when used with insulin or sulfonylureas.
Iodinated contrast media, alcohol, insulin and sulfonylureas (increased hypoglycaemia risk), corticosteroids and thiazide/loop diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), cimetidine (may increase metformin levels), ACE inhibitors (may enhance glucose-lowering), carbonic anhydrase inhibitors like topiramate/acetazolamide (increased acidosis risk)
