Management of type 2 diabetes mellitus in adults to improve blood glucose control along with diet and exercise; used when metformin alone is inadequate or in patients already stabilized on vildagliptin and metformin.
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon secretion. Metformin reduces hepatic glucose production and improves insulin sensitivity, thereby lowering blood glucose.
Tablet for oral use. Take as prescribed, usually with food to reduce stomach upset. Swallow whole (do not crush/chew if modified release). Maintain regular timing; monitor blood glucose as advised.

Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increased with kidney disease, dehydration, severe infection, liver disease, heart failure, heavy alcohol use; stop and seek care if rapid breathing, severe weakness, abdominal pain, or unusual sleepiness occur. Assess renal function before and during therapy; dose adjustment/avoid in significant renal impairment. Vildagliptin can affect liver enzymes—periodic LFT monitoring recommended; stop if jaundice or persistent elevations occur. Temporarily withhold metformin around iodinated contrast procedures or major surgery as advised by a doctor.
Alcohol, iodinated contrast media, insulin/sulfonylureas (increased hypoglycaemia risk), ACE inhibitors (may increase hypoglycaemia risk), corticosteroids (may raise glucose), thiazide/loop diuretics (may raise glucose or worsen dehydration/renal function), cimetidine (may increase metformin levels), carbonic anhydrase inhibitors like topiramate/acetazolamide (may increase acidosis risk)
