Treatment of type 2 diabetes mellitus to improve blood sugar control along with diet and exercise; used when single medicine is insufficient.

Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1/GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin decreases hepatic glucose production and improves insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia when used alone.

Oral tablet; take exactly as prescribed, usually with meals to reduce stomach upset (especially due to metformin). Swallow whole with water; do not crush/chew. Follow diet/exercise advice and monitor blood glucose regularly.

Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, upper respiratory tract infection, hypoglycaemia (more likely with other antidiabetics), low vitamin B12 (long-term)

Prescription medicine. Risk of lactic acidosis with metformin (higher in kidney disease, dehydration, severe infection, heavy alcohol use, liver disease, or low oxygen states); stop and seek care if rapid breathing, severe weakness, abdominal pain, or unusual sleepiness occurs. Check kidney function before and during therapy; not recommended in severe renal impairment. Temporarily stop metformin before/after iodinated contrast procedures or major surgery as advised. Monitor for pancreatitis symptoms (severe abdominal pain), allergic reactions, and liver enzyme elevation; periodic LFTs may be advised. Use caution in elderly and in conditions causing dehydration.

Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate/acetazolamide), cimetidine, diuretics, ACE inhibitors, corticosteroids, thyroid hormones, sympathomimetics, other antidiabetics (insulin/sulfonylureas increase hypoglycaemia risk)

Store below 25°C in a dry place; protect from moisture and light; keep in original pack and out of reach of children.
