Type 2 diabetes mellitus: to improve blood glucose control in adults when diet and exercise are not sufficient; used when dual therapy with metformin and a DPP-4 inhibitor is appropriate
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin decreases hepatic glucose production, reduces intestinal glucose absorption, and improves insulin sensitivity, lowering blood sugar levels.
Oral tablet. Take as prescribed, usually once or twice daily with meals to reduce stomach upset (especially due to metformin). Swallow whole; do not crush/chew if SR/ER. Monitor blood glucose regularly and follow diet/exercise advice.
Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, upper respiratory tract infection, nasopharyngitis
Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk higher with kidney impairment, dehydration, severe infection, heart failure, alcohol use, or liver disease; dose adjustment/avoid in renal impairment (check eGFR). Stop temporarily before iodinated contrast procedures and during acute serious illness/surgery as advised by doctor. Vildagliptin: monitor liver enzymes; use caution in hepatic impairment and if symptoms of hepatitis occur. Risk of hypoglycaemia increases when combined with insulin or sulfonylureas. Seek help for severe abdominal pain (pancreatitis) or allergic reactions.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cationic drugs affecting renal tubular secretion (e.g., cimetidine), insulin/sulfonylureas (increased hypoglycaemia), ACE inhibitors (may increase risk of angioedema with DPP-4 inhibitors), diuretics/corticosteroids/thyroid hormones (may worsen glycaemic control)
