Type 2 diabetes mellitus (to improve blood sugar control) when diet and exercise are not enough; used alone or with other antidiabetic medicines as prescribed.

The brand name suggests a vildagliptin + metformin combination. Vildagliptin (a DPP-4 inhibitor) increases incretin levels, which increases insulin release and decreases glucagon in a glucose-dependent manner. Metformin reduces glucose production in the liver and improves insulin sensitivity, lowering blood sugar.

Take exactly as prescribed by your doctor. If this contains metformin, it is usually taken with meals to reduce stomach upset; swallow tablets whole with water and do not crush/chew if sustained/modified release. Maintain diet, exercise, and regular blood sugar monitoring.

Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, headache, dizziness, upper respiratory tract infection, low blood sugar (especially with other antidiabetics)

Do not use in type 1 diabetes or diabetic ketoacidosis. If the product contains metformin: risk of lactic acidosis—seek urgent care for rapid breathing, severe weakness, drowsiness, or abdominal pain; avoid in severe kidney disease and use caution in liver disease, dehydration, severe infection, or heavy alcohol use. Monitor kidney function periodically; temporarily stop before/after iodinated contrast studies and major surgery as advised. If the product contains vildagliptin: monitor liver enzymes; stop and consult doctor if jaundice, dark urine, persistent nausea, or abdominal pain; rare pancreatitis—seek care for severe persistent abdominal pain.

Alcohol (increases risk of hypoglycaemia and lactic acidosis with metformin), insulin or sulfonylureas (increased hypoglycaemia risk), iodinated contrast media (metformin—lactic acidosis risk), diuretics/ACE inhibitors/ARBs/NSAIDs (may affect kidney function), cimetidine (may increase metformin levels), corticosteroids and some antipsychotics (may raise blood sugar)
