Monograph
Human insulin premix 50/50 injection: Insulin soluble (regular human insulin) 50% + Insulin isophane (NPH) 50%; strength 40 IU/mL
Treatment of diabetes mellitus (Type 1 and Type 2) requiring insulin; helps control high blood sugar and prevent complications of diabetes
This is a premixed human insulin containing short-acting regular insulin and intermediate-acting NPH insulin. Regular insulin covers rise in glucose after meals, while NPH provides basal control for several hours to maintain glucose levels between meals and overnight.
For subcutaneous injection only, as prescribed by the doctor. Usually injected 30 minutes before meals; timing and dose depend on blood sugar, diet and activity. Roll the vial gently to mix (do not shake); use insulin syringe calibrated for 40 IU/mL. Rotate injection sites (abdomen, thigh, upper arm) and monitor blood glucose regularly.
Hypoglycaemia (low blood sugar), injection site reactions (pain/redness/swelling/itching), weight gain, lipodystrophy at injection site, oedema, allergic reactions (rash/urticaria)
Risk of hypoglycaemia—carry glucose/sugar; avoid skipping meals after dosing. Use caution in renal/hepatic impairment, elderly, and during changes in diet/exercise; dose adjustments may be needed. Do not use if insulin appears clumped/frosted or if vial is damaged/expired. Not for intravenous use unless specifically directed by a specialist; do not mix with other insulins unless advised. Seek urgent care for severe hypoglycaemia or allergic reactions.
Other antidiabetics (sulfonylureas, metformin) may increase hypoglycaemia risk; beta-blockers may mask hypoglycaemia symptoms; corticosteroids, thiazide diuretics, thyroid hormones, sympathomimetics may increase blood glucose; ACE inhibitors and alcohol can increase hypoglycaemia risk; MAO inhibitors may enhance insulin effect.
Store unopened vials at 2–8°C (refrigerator); do not freeze. Protect from light. After opening, keep as per label instructions (commonly below 25–30°C or refrigerated) and use within the recommended in-use period; keep away from heat and direct sunlight.
