Treatment of gastroesophageal reflux disease (GERD), heartburn and acid regurgitation, erosive esophagitis, gastric and duodenal ulcers, prevention/treatment of NSAID-induced ulcers, Zollinger-Ellison syndrome and other pathological hypersecretory conditions
Pantoprazole is a proton pump inhibitor (PPI). It irreversibly blocks the H+/K+ ATPase (proton pump) in gastric parietal cells, thereby reducing stomach acid production and allowing ulcers/erosions to heal.
Oral tablet. Take as advised by doctor, usually 1 tablet once daily 30–60 minutes before food (preferably before breakfast). Swallow whole; do not crush, break or chew the enteric-coated tablet. Duration depends on condition.
Headache, diarrhea, nausea, abdominal pain, flatulence, dizziness, vomiting, constipation
Use with caution in severe liver disease. Long-term use may increase risk of vitamin B12 deficiency, low magnesium, fractures (osteoporosis), and gastric polyps; use the lowest effective dose. Seek medical advice if alarm symptoms occur (unexplained weight loss, persistent vomiting, difficulty swallowing, blood in vomit/stool, black stools). May rarely cause severe skin reactions or kidney inflammation (interstitial nephritis). Can reduce absorption of drugs needing acidic pH.
Atazanavir/nelfinavir (reduced effect; avoid), warfarin (monitor INR), methotrexate (high-dose; increased levels), digoxin/diuretics (risk of hypomagnesemia), ketoconazole/itraconazole/erlotinib/iron salts (reduced absorption), clopidogrel (possible reduced antiplatelet effect; consider alternatives/monitor), tacrolimus (increased levels possible)
