Sitagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), leading to increased insulin release and reduced glucagon in a glucose-dependent manner. Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity, reducing blood glucose without usually causing hypoglycaemia when used alone.
Take orally as directed by a physician, usually with meals to reduce stomach upset. Swallow tablet whole with water; do not crush/chew unless advised. Take at the same time each day; dose is individualized based on glucose control and kidney function.

Prescription-only. Monitor kidney function before and during therapy; metformin is contraindicated in severe renal impairment and should be used with caution in reduced eGFR. Risk of lactic acidosis (rare but serious), higher with kidney disease, dehydration, severe infection, hypoxia, liver disease, heavy alcohol use; stop and seek urgent care if rapid breathing, severe weakness, abdominal pain occur. Temporarily stop metformin around iodinated contrast procedures and major surgery as advised. Sitagliptin may rarely cause pancreatitis and severe joint pain; discontinue and consult doctor if severe abdominal pain. Monitor for hypersensitivity reactions and signs of heart failure in susceptible patients.
Alcohol (increases lactic acidosis risk), iodinated contrast media (temporary interruption), carbonic anhydrase inhibitors e.g. topiramate/acetazolamide (increases metabolic acidosis risk), cationic drugs affecting renal tubular secretion (e.g. cimetidine), insulin and sulfonylureas (increased hypoglycaemia risk; dose adjustment may be needed), diuretics/ACE inhibitors/NSAIDs (may affect renal function), digoxin (sitagliptin may slightly increase digoxin levels)
