Type 2 diabetes mellitus: to improve glycaemic control in adults when diet and exercise alone or metformin/sitagliptin monotherapy is inadequate.

Sitagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), leading to glucose-dependent increased insulin release and reduced glucagon. Metformin decreases hepatic glucose production and improves insulin sensitivity, reducing glucose absorption and improving peripheral uptake.

Take exactly as prescribed. Usually taken orally with meals to reduce stomach upset; swallow tablet whole with water. Do not crush/chew unless advised. Dose is individualized based on kidney function and blood sugar; follow your doctor’s schedule.

Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, upper respiratory tract infection, nasopharyngitis

Prescription only. Risk of lactic acidosis with metformin (medical emergency): higher risk in kidney impairment, dehydration, severe infection, hypoxia, heart failure, liver disease, or excess alcohol; stop and seek help if fast breathing, severe weakness, muscle pain, abdominal pain, or drowsiness occur. Assess renal function before and during therapy; dose adjustment/avoid in significant renal impairment. Temporarily stop metformin before iodinated contrast procedures and major surgery as advised. Sitagliptin: rare pancreatitis and severe joint pain; stop and consult if severe persistent abdominal pain. Monitor for hypoglycaemia if used with insulin/sulfonylureas.

Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cationic drugs that reduce metformin clearance (e.g., cimetidine), insulin/sulfonylureas (increased hypoglycaemia risk), digoxin (sitagliptin may slightly increase levels)

Store below 25°C, protected from moisture and light. Keep in original blister pack and out of reach of children.
