Composition of Rablet L 40 Capsule

Rablet L 40 Capsule contains Rabeprazole 20 mg + Levosulpiride (sustained release) 75 mg. It belongs to the Proton Pump Inhibitor (Rabeprazole) + D2 receptor antagonist prokinetic (Levosulpiride) class of medicines.

How Rablet L 40 Capsule works

Rabeprazole is a proton pump inhibitor (PPI) that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Levosulpiride is a dopamine D2 receptor antagonist with prokinetic action that improves gastric motility and speeds gastric emptying, helping reduce reflux and dyspeptic symptoms.

How to take Rablet L 40 Capsule

Capsule (SR). Take exactly as prescribed, usually once daily, preferably 30 minutes before food (often before breakfast). Swallow whole; do not crush or open the capsule. If symptoms persist, consult your doctor.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine may cause confusion, blurred vision, sweating and a fast heartbeat. It can cause drowsiness, disorientation and abnormal muscle movements. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Rablet L 40 Capsule

Most side effects of Rablet L are mild and settle as the body adjusts. Reported side effects include:

• diarrhea
• constipation
• abdominal pain
• nausea
• dizziness
• sleepiness
• dry mouth
• increased prolactin (breast tenderness/galactorrhea)
• restlessness

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Use with caution in liver disease; may cause dizziness/sleepiness—avoid driving if affected. Long-term PPI use may increase risk of low magnesium, vitamin B12 deficiency, fractures and infections (e.g., C. difficile). Seek medical advice if you have alarm symptoms (vomiting blood, black stools, unexplained weight loss, persistent vomiting, difficulty swallowing). Levosulpiride may cause movement disorders/extrapyramidal symptoms, hyperprolactinemia, and rarely QT prolongation—use caution in patients with heart rhythm disorders. Not recommended in patients with suspected/known GI obstruction, perforation or bleeding.

Do not take Rablet L if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for people with bleeding, blockage or perforation of the gut, or with certain heart rhythm disorders.

Interactions with other medicines

Warfarin/other anticoagulants (monitor), digoxin (risk increased with low magnesium), methotrexate (high dose—PPI may increase levels), ketoconazole/itraconazole/posaconazole/erlotinib (reduced absorption due to low acid), clopidogrel (possible reduced activation—prefer medical advice), other QT-prolonging drugs (e.g., amiodarone, sotalol, certain macrolides/fluoroquinolones, antipsychotics), levodopa/dopamine agonists (reduced effect), CNS depressants/alcohol (additive sedation)

How to store Rablet L 40 Capsule

Store below 25°C in a dry place, protected from light and moisture. Keep in original blister pack. Keep out of reach of children.

Expert advice for Rablet L 40 Capsule

Take it before breakfast, not after a meal. Eat smaller meals, avoid late dinners, raise the head of your bed and avoid smoking — these do as much as the tablet for reflux. Take it shortly before meals for bloating and early fullness. Sip fluids frequently in small amounts, and use oral rehydration salts if you have been vomiting.
