Treatment of moderate to severe rheumatoid arthritis in adults (when response to DMARDs like methotrexate is inadequate), psoriatic arthritis in adults, and moderate to severe ulcerative colitis in adults (as prescribed by specialist).
Tofacitinib is a Janus kinase (JAK) inhibitor that reduces inflammatory signaling by inhibiting JAK enzymes involved in cytokine pathways. This lowers immune-mediated inflammation and helps reduce joint pain/swelling and gut inflammation in eligible patients.
Oral tablet (extended-release). Take 1 tablet once daily, preferably at the same time each day, with or without food. Swallow whole; do not crush, break, or chew. Use only as prescribed; regular lab monitoring is required.
Upper respiratory tract infection, headache, diarrhea, nausea, nasopharyngitis, increased cholesterol, elevated liver enzymes, hypertension, rash
Prescription-only immunosuppressant. Increases risk of serious infections (including TB, fungal and opportunistic infections); screen for TB/hepatitis and monitor. Risk of blood clots (DVT/PE), major cardiovascular events and malignancy may be increased in certain patients; use lowest effective dose and assess risk factors (age, smoking, cardiovascular history). Monitor CBC (lymphocytes/neutrophils/hemoglobin), liver enzymes, lipids; dose adjustments/avoid in severe hepatic impairment and severe renal impairment as advised. Avoid live vaccines during and shortly after treatment. Not recommended in active serious infections; seek urgent care for fever, cough, breathlessness, chest pain, leg swelling, or unusual bleeding/bruising.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase levels; strong CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin, St John’s wort) may reduce effect; moderate CYP3A4 inhibitors with strong CYP2C19 inhibitors (e.g., fluconazole) increase exposure; other immunosuppressants/biologics (e.g., azathioprine, cyclosporine, tacrolimus, TNF inhibitors) increase infection risk; live vaccines should be avoided.
