Pantoprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Domperidone is a dopamine (D2) receptor antagonist that increases gastric motility and has anti-nausea effects by enhancing gastric emptying and reducing reflux.
Capsule: Take as prescribed, usually 1 capsule once daily, preferably 30 minutes before breakfast. Swallow whole; do not crush or chew (DSR/modified release).
Headache, diarrhea, abdominal pain, nausea, dry mouth, dizziness, flatulence, constipation
Use with caution in liver disease; long-term PPI use may increase risk of vitamin B12 deficiency, low magnesium, fractures and infections (e.g., C. difficile). Domperidone may cause QT prolongation/serious heart rhythm problems—avoid in known QT prolongation, significant cardiac disease, or with other QT-prolonging medicines; seek care for palpitations or fainting. Not recommended in severe hepatic impairment; use under medical supervision in elderly. Stop and consult doctor if black stools, vomiting blood, or persistent/worsening symptoms.
Domperidone: strong CYP3A4 inhibitors (ketoconazole/itraconazole, clarithromycin/erythromycin, ritonavir) and other QT-prolonging drugs (amiodarone, sotalol, certain antipsychotics/fluoroquinolones) increase arrhythmia risk. Pantoprazole: may reduce absorption of drugs needing acidic pH (ketoconazole, atazanavir); may affect warfarin/INR (monitor), high-dose methotrexate (avoid/monitor), digoxin (with hypomagnesemia).
