Type 2 diabetes mellitus when diet and exercise alone or dual therapy is insufficient; helps control post-meal and fasting blood glucose.
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin SR improves insulin sensitivity and reduces glucose production by the liver. Voglibose inhibits intestinal alpha-glucosidase enzymes, delaying carbohydrate absorption and reducing post-meal glucose spikes.
Oral tablet. Take exactly as prescribed, typically once daily with/after a main meal (often breakfast) to reduce stomach upset and risk of low sugar. Swallow whole (do not crush/chew) due to sustained-release metformin; follow diet and exercise advice and monitor blood glucose.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, flatulence, bloating, headache, dizziness
May cause hypoglycaemia (especially if meals are skipped, with alcohol, or with other antidiabetics); carry a quick source of sugar. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, severe infection, dehydration, heavy alcohol use, or heart/liver failure; kidney function monitoring advised. Temporarily stop metformin around iodinated contrast procedures or major surgery as advised by doctor. Use caution in elderly and in people with renal/hepatic impairment. Voglibose may cause significant GI gas/diarrhoea; avoid in intestinal obstruction or severe digestive disorders. Seek medical help for severe weakness, breathing difficulty, persistent vomiting, or signs of pancreatitis (severe abdominal pain).
Alcohol, insulin/other antidiabetics (increased hypoglycaemia), beta-blockers (mask hypoglycaemia), corticosteroids and thiazide diuretics (raise glucose), ACE inhibitors (may increase hypoglycaemia), rifampicin (reduces sulfonylurea effect), azole antifungals/clarithromycin (increase sulfonylurea levels), iodinated contrast media (metformin-related lactic acidosis risk), cimetidine (increases metformin levels)
