Treatment of fungal infections such as dermatophytosis (tinea/ringworm), pityriasis versicolor, oral/vaginal candidiasis, and other susceptible systemic fungal infections as prescribed.
Itraconazole is a triazole antifungal that inhibits fungal cytochrome P450 (14-alpha demethylase), blocking ergosterol synthesis. This disrupts the fungal cell membrane, stopping growth and leading to fungal cell death.
Take the capsule exactly as prescribed. Usually taken after food for better absorption; swallow whole with water. Duration depends on type/site of infection—complete the full course even if symptoms improve.
Nausea, abdominal pain, diarrhea, headache, dizziness, rash, elevated liver enzymes
Prescription-only antifungal. Use with caution in liver disease; monitor for symptoms of hepatitis (yellowing of eyes/skin, dark urine, persistent nausea). Can worsen or precipitate heart failure—avoid/use with caution in patients with ventricular dysfunction or heart failure. Seek urgent care for severe allergic reactions, severe skin rash, or breathing difficulty.
Avoid with drugs that prolong QT or are strongly affected by CYP3A4 inhibition. Important interactions include: simvastatin/lovastatin, midazolam/triazolam (oral), ergot alkaloids, cisapride, quinidine/dofetilide, some antiarrhythmics, rifampicin/rifabutin (reduce effect), phenytoin/carbamazepine (reduce effect), warfarin (bleeding risk), certain calcium channel blockers, digoxin, ciclosporin/tacrolimus/sirolimus, some HIV protease inhibitors. Acid suppressants (PPIs/H2 blockers/antacids) may reduce absorption—separate dosing if advised.
