Treatment of type 2 diabetes mellitus to improve blood glucose control, as monotherapy or in combination with other antidiabetic medicines (e.g., metformin, sulfonylureas, insulin) along with diet and exercise
Linagliptin is a DPP-4 inhibitor that increases levels of incretin hormones (GLP-1 and GIP). This enhances glucose-dependent insulin release and reduces glucagon secretion, thereby lowering blood sugar with a low risk of hypoglycaemia when used alone.
Take by mouth as prescribed, typically 5 mg once daily, with or without food. Swallow tablet whole with water at the same time each day; continue diet and exercise. If a dose is missed, take it when remembered (do not double the dose).
Nasopharyngitis, cough, headache, diarrhoea, nausea, dizziness, hypoglycaemia (especially with sulfonylurea/insulin), skin rash
Not for type 1 diabetes or diabetic ketoacidosis. Risk of pancreatitis—seek urgent care for severe persistent abdominal pain (with/without vomiting). Hypoglycaemia risk increases when combined with insulin or sulfonylureas—dose adjustment may be needed. Rare severe allergic reactions (angioedema, anaphylaxis) and serious skin reactions—stop and seek medical help. Use caution in patients with history of pancreatitis. Monitor blood glucose and HbA1c regularly.
Insulin and sulfonylureas (increased hypoglycaemia risk), rifampicin and other strong P-gp/CYP3A4 inducers (may reduce linagliptin effect), carbamazepine/phenytoin/phenobarbital (may reduce effect), digoxin (generally no clinically significant interaction but monitor as advised)
