Rabeprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Levosulpiride is a prokinetic (dopamine D2 antagonist) that improves gastric motility and speeds stomach emptying, helping reduce reflux, fullness and nausea.
Capsule for oral use. Usually taken once daily, preferably 30 minutes before food (commonly before breakfast), or as directed by the physician. Swallow whole; do not crush or chew (SR component).
Headache, diarrhea, abdominal pain, nausea, constipation, dizziness, sleepiness, dry mouth, increased prolactin (breast tenderness/galactorrhea), restlessness
Use only on medical advice. Long-term PPI use may be associated with low magnesium, vitamin B12 deficiency and increased fracture risk; report persistent diarrhea (possible C. difficile). Levosulpiride may cause extrapyramidal symptoms, drowsiness, and raised prolactin; use caution in Parkinson’s disease, epilepsy, history of movement disorders, and in patients with prolactin-dependent tumors. Caution in severe liver/kidney disease. Seek medical help for alarm symptoms (vomiting blood, black stools, unexplained weight loss, persistent vomiting/difficulty swallowing).
Rabeprazole may reduce absorption of drugs requiring acidic pH (ketoconazole/itraconazole, iron salts) and can increase levels of digoxin; may interact with high-dose methotrexate and some antiretrovirals (e.g., atazanavir). Levosulpiride may have additive sedation with alcohol/CNS depressants; antagonizes levodopa/dopamine agonists; increased risk of QT prolongation/arrhythmia with other QT-prolonging drugs (certain antiarrhythmics, macrolides, fluoroquinolones, antipsychotics); antacids may reduce prokinetic effect if taken together.
