Treatment of chronic heart failure (especially heart failure with reduced ejection fraction, HFrEF) to reduce risk of cardiovascular death and hospitalization

Sacubitril inhibits neprilysin, increasing levels of natriuretic peptides which promote vasodilation, natriuresis and reduced cardiac remodeling. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.

Tablet for oral use. Take exactly as prescribed, usually twice daily. Can be taken with or without food; take at the same times each day. Do not stop abruptly without medical advice. A 36-hour washout is required when switching from an ACE inhibitor to sacubitril/valsartan.

Low blood pressure (dizziness), increased potassium (hyperkalemia), impaired kidney function/raised creatinine, cough, headache, fatigue, diarrhea, nausea

Contraindicated with concomitant ACE inhibitors and in patients with history of angioedema related to ACE inhibitor/ARB; maintain 36-hour gap after stopping ACE inhibitor. Avoid in pregnancy (fetal toxicity) and generally not recommended during breastfeeding. Monitor blood pressure, kidney function and serum potassium, especially in elderly, dehydration, renal impairment, diabetes, or when used with diuretics. Use caution in severe hepatic impairment; may cause angioedema—seek urgent care if swelling of face/lips/tongue or breathing difficulty occurs.

ACE inhibitors (contraindicated; 36-hour washout), other ARBs/aliskiren (avoid; especially in diabetes), potassium supplements/salt substitutes and potassium-sparing diuretics (hyperkalemia), NSAIDs including COX-2 inhibitors (worsen renal function), lithium (increased lithium levels/toxicity), diuretics and other antihypertensives (additive hypotension)

Store below 30°C, in original pack, protected from moisture and light. Keep out of reach of children.
