Treatment of lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH) such as weak urine stream, hesitancy, incomplete bladder emptying; management of overactive bladder symptoms (urgency, frequency, urge incontinence) when present along with BPH/LUTS.
Silodosin is a selective alpha-1A adrenergic blocker that relaxes smooth muscle in the prostate and bladder neck to improve urine flow and reduce obstruction symptoms. Mirabegron is a beta-3 adrenergic agonist that relaxes the bladder detrusor during the storage phase, increasing bladder capacity and reducing urgency/frequency.
Take as prescribed by the doctor. Usually taken once daily with food, preferably at the same time each day. Swallow the tablet whole with water; do not crush or chew. If a dose is missed, take it when remembered unless it is close to the next dose; do not double.
Dizziness, headache, orthostatic hypotension (drop in BP on standing), nasal congestion, retrograde ejaculation/ejaculation disorders, diarrhea, nausea, constipation, dry mouth, palpitations, increased blood pressure, urinary retention
For men with BPH/LUTS: may cause dizziness/fainting due to low blood pressure—rise slowly and use caution when driving. Mirabegron can increase blood pressure; monitor BP especially in patients with hypertension and avoid/use with caution in severe uncontrolled hypertension. Risk of urinary retention, especially in bladder outlet obstruction and when combined with antimuscarinics—seek help if unable to pass urine. Dose adjustment/caution may be needed in kidney or liver impairment; avoid strong CYP3A4 inhibitors with silodosin in some patients. Tell eye surgeon before cataract/glaucoma surgery (risk of intraoperative floppy iris syndrome with alpha-blockers).
Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) may increase silodosin levels; other alpha-blockers or antihypertensives may increase risk of low BP; PDE5 inhibitors (sildenafil, tadalafil) may enhance hypotensive effect; mirabegron (CYP2D6 inhibitor) can increase levels of metoprolol, desipramine, flecainide, propafenone, thioridazine; digoxin levels may increase (monitor); caution with drugs that raise BP.
Store below 25°C in a dry place, protected from light and moisture. Keep in original blister pack and out of reach of children.
