Type 2 diabetes mellitus (to improve glycaemic control along with diet and exercise), especially when metformin alone is inadequate

Sitagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), boosting glucose-dependent insulin release and reducing glucagon. Metformin decreases hepatic glucose production and improves insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia when used alone.

Oral tablet. Take as prescribed, usually twice daily with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Follow diet/exercise plan and monitor blood glucose regularly.

Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, upper respiratory tract infection, nasopharyngitis

Not for type 1 diabetes or diabetic ketoacidosis. Metformin can rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, heart failure, heavy alcohol use, or hypoxia; assess renal function before and during therapy. Stop temporarily before/after iodinated contrast or major surgery as advised. Sitagliptin may rarely cause pancreatitis, severe joint pain, allergic reactions, or bullous pemphigoid—seek care if severe abdominal pain or rash. Use caution in elderly and in hepatic impairment.

Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cationic drugs affecting renal tubular secretion (e.g., cimetidine), diuretics, ACE inhibitors/ARBs, corticosteroids, thyroid hormones, sympathomimetics, oral contraceptives, other antidiabetics/insulin (increased hypoglycaemia risk)
