Treatment of rheumatoid arthritis (moderate to severe) in adults, psoriatic arthritis in adults, ulcerative colitis (moderate to severe) in adults (as prescribed by specialist)
Tofacitinib is a Janus kinase (JAK) inhibitor. It blocks JAK-mediated signaling of inflammatory cytokines, thereby reducing immune activation, inflammation, pain, and joint/intestinal symptoms.
Tablet for oral use. Take exactly as prescribed by the doctor, usually 5 mg once or twice daily depending on indication. Can be taken with or without food; swallow whole with water at the same time each day. Do not stop or change dose without medical advice.
Upper respiratory tract infection, headache, nausea, diarrhea, increased cholesterol, elevated liver enzymes, hypertension, herpes zoster (shingles), urinary tract infection
Prescription-only immunosuppressant. Increased risk of serious infections (TB, fungal, bacterial, viral) and reactivation of TB/hepatitis; screening may be needed before starting. Risk of blood clots (DVT/PE), major cardiovascular events, and malignancy may increase in some patients—use lowest effective dose and follow specialist advice. Monitor CBC, liver enzymes, lipids; watch for signs of infection. Use caution in liver/kidney impairment. Avoid live vaccines during treatment.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) may increase levels; strong CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin) may reduce effect. Immunosuppressants/biologics (e.g., azathioprine, cyclosporine, tacrolimus, TNF inhibitors) increase infection risk—generally avoid combination. Live vaccines should be avoided.
Store at room temperature (below 25°C), in a dry place, protected from light and moisture. Keep in original blister pack and out of reach of children.
