Treatment of moderate to severe rheumatoid arthritis (RA) in adults (especially when response to methotrexate is inadequate/intolerant); may be used alone or with methotrexate/other non-biologic DMARDs. Also used in certain inflammatory conditions as prescribed by a specialist (e.g., psoriatic arthritis, ulcerative colitis) based on clinical judgement and approval status.
Tofacitinib is a Janus kinase (JAK) inhibitor (primarily JAK1/JAK3) that reduces signaling of multiple cytokines involved in inflammation. This decreases immune overactivity and helps reduce joint pain, swelling, and progression of inflammatory damage.
Oral tablet. Take exactly as prescribed by the doctor, usually 5 mg twice daily (or as advised). Can be taken with or without food; swallow whole with water. Do not stop abruptly without medical advice; regular lab monitoring is required.
Upper respiratory tract infection, headache, diarrhea, nausea, increased cholesterol, hypertension, rash, herpes zoster (shingles), anemia, neutropenia/low white blood cells, elevated liver enzymes
Serious risk of infections (including TB, fungal, and opportunistic infections); screen for TB and hepatitis before starting. Increased risk of herpes zoster. May increase risk of blood clots (DVT/PE), major cardiovascular events, and malignancies (including lymphoma and skin cancers), especially in older patients and those with risk factors. Monitor CBC, liver function, lipids; avoid in severe hepatic impairment; dose adjustments may be needed in renal/hepatic impairment. Not recommended with other potent immunosuppressants/biologics. Seek urgent care for fever, persistent cough, shortness of breath, chest pain, leg swelling, or unexplained bruising/bleeding.
Strong CYP3A4 inhibitors (e.g., ketoconazole) and moderate CYP3A4 + strong CYP2C19 inhibitors (e.g., fluconazole) may increase levels; strong CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin) may reduce effect. Increased infection risk with biologic DMARDs (e.g., adalimumab, etanercept, infliximab), other immunosuppressants (e.g., azathioprine, cyclosporine, tacrolimus). Live vaccines should be avoided during therapy.
Store at room temperature (below 25°C), protected from moisture and light. Keep in original blister pack and out of reach of children.
