Type 2 diabetes mellitus (adjunct to diet and exercise) to help control post-meal (postprandial) and fasting blood glucose, especially when single therapy is inadequate
Voglibose is an alpha-glucosidase inhibitor that slows carbohydrate digestion in the intestine, reducing post-meal glucose spikes. Metformin improves insulin sensitivity and decreases glucose production by the liver, helping lower overall blood sugar.
Take orally exactly as prescribed. Typically, voglibose-containing tablets are taken with the first bite of each main meal; metformin is best taken with food to reduce stomach upset. Swallow with water; do not crush/chew unless advised. Dose is individualized based on glucose control and tolerability.
Flatulence, abdominal discomfort, diarrhea, nausea, vomiting, loss of appetite, metallic taste, headache, dizziness
For prescription use in type 2 diabetes only. Metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, unusual sleepiness, abdominal pain, or muscle cramps; risk increases with kidney disease, dehydration, sepsis, heavy alcohol use, and severe heart/liver disease. Avoid in severe renal impairment; kidney function should be assessed before and during treatment. Hold metformin temporarily around iodinated contrast imaging or major surgery as advised. Voglibose commonly causes gas/diarrhea; avoid in inflammatory bowel disease, intestinal obstruction, or chronic intestinal disorders. Monitor blood glucose and HbA1c; risk of hypoglycemia increases if used with insulin/sulfonylureas (treat low sugar with glucose/dextrose, not sucrose).
Alcohol (increases lactic acidosis risk), iodinated contrast media (temporary stop metformin), insulin and sulfonylureas (increased hypoglycemia risk), cimetidine (may increase metformin levels), carbonic anhydrase inhibitors e.g. topiramate/acetazolamide (increase metabolic acidosis risk), diuretics/ACE inhibitors/ARBs/NSAIDs (may affect kidney function), corticosteroids and sympathomimetics (may raise blood glucose)
