Treatment of chronic heart failure with reduced ejection fraction (HFrEF) to reduce the risk of cardiovascular death and hospitalization for heart failure; used in place of an ACE inhibitor or ARB as advised by doctor.

Sacubitril inhibits neprilysin, increasing levels of beneficial natriuretic peptides that promote vasodilation and sodium excretion. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.

Oral tablet. Take exactly as prescribed, usually twice daily. Can be taken with or without food; take at the same times each day. Do not start within 36 hours of stopping an ACE inhibitor; do not stop or change dose without medical advice.

Low blood pressure (dizziness), increased potassium (hyperkalemia), kidney function changes, cough, diarrhea, nausea, fatigue, headache

Contraindicated with ACE inhibitors (need 36-hour washout) and in history of angioedema related to ACEi/ARB. Do not use with aliskiren in patients with diabetes. Monitor blood pressure, kidney function and serum potassium, especially in elderly, dehydrated patients, and those with renal impairment. Risk of angioedema—seek urgent care for swelling of face/lips/tongue or breathing difficulty. Not recommended in pregnancy; avoid in severe hepatic impairment; caution in renal artery stenosis and volume depletion.

ACE inhibitors (contraindicated; 36-hour gap required), aliskiren (avoid in diabetes), potassium supplements/salt substitutes and potassium-sparing diuretics (e.g., spironolactone/eplerenone/amiloride) (hyperkalemia risk), NSAIDs (may worsen kidney function), lithium (increased lithium toxicity), other antihypertensives and diuretics (additive hypotension)

Store at room temperature (below 30°C), protected from moisture and light. Keep in original blister pack and out of reach of children.
