Type 2 diabetes mellitus (as an add-on to diet and exercise) to improve glycaemic control in adults when dual therapy is appropriate.

Sitagliptin is a DPP-4 inhibitor that increases incretin hormones, thereby increasing insulin release and decreasing glucagon levels in a glucose-dependent manner. Dapagliflozin is an SGLT2 inhibitor that lowers blood glucose by increasing urinary glucose excretion through the kidneys.

Tablet for oral use. Take as prescribed by the doctor, usually once daily; may be taken with or without food, preferably at the same time each day. Maintain adequate hydration; follow diet/exercise advice and monitor blood glucose.

Genital fungal infection,urinary tract infection,increased urination,thirst,headache,nasopharyngitis,dizziness,nausea,constipation,low blood sugar (especially with insulin/sulfonylurea)

Not for type 1 diabetes or diabetic ketoacidosis. Risk of dehydration and low blood pressure (more in elderly/diuretics); maintain fluids. SGLT2 inhibitors may increase risk of genital mycotic infections, UTIs and rare Fournier’s gangrene—seek care for genital pain/swelling/fever. Rare risk of pancreatitis with sitagliptin—stop and consult if severe abdominal pain occurs. Monitor kidney function before and during therapy; use with caution/avoid in significant renal impairment as advised by physician. Use caution if history of recurrent UTIs, ketoacidosis, alcoholism, low-carb intake, or surgery/acute illness (ketoacidosis risk).

Insulin and sulfonylureas (increased hypoglycaemia risk),diuretics (increased dehydration/hypotension risk),rifampicin and other strong enzyme inducers (may reduce sitagliptin effect),digoxin (sitagliptin may increase levels),other glucose-lowering drugs (additive effect)
