Type 2 diabetes mellitus: used to improve blood glucose control along with diet and exercise when dual therapy with metformin and a DPP-4 inhibitor is appropriate.
Sitagliptin is a DPP-4 inhibitor that increases incretin hormones, leading to increased insulin release and reduced glucagon secretion in a glucose-dependent manner. Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity, reducing blood glucose levels.
Oral tablet. Take as prescribed by doctor, usually once daily with the evening meal (for SR/ER) or with meals to reduce stomach upset. Swallow whole; do not crush/chew if sustained/extended release. Monitor blood sugar regularly.

Prescription-only antidiabetic. Risk of lactic acidosis with metformin (higher risk in kidney impairment, dehydration, severe infection, low oxygen states, heavy alcohol use); stop and seek urgent care for deep/rapid breathing, severe weakness, abdominal pain. Not recommended in severe renal impairment; dose adjustment/avoid based on eGFR. Hold metformin before/after iodinated contrast procedures and major surgery as advised. Sitagliptin may rarely cause pancreatitis or severe joint pain; stop and consult doctor if severe abdominal pain. Use caution in elderly; monitor kidney function and vitamin B12 with long-term use. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases lactic acidosis risk),iodinated contrast media,carbonic anhydrase inhibitors (e.g., topiramate/acetazolamide),cimetidine,diuretics/ACE inhibitors/NSAIDs (may worsen renal function),insulin or sulfonylureas (increased hypoglycaemia risk),digoxin (may increase levels with sitagliptin)
