Type 2 diabetes mellitus: to improve glycaemic control in adults when diet and exercise plus other antidiabetic medicines are not adequate
Sitagliptin (DPP-4 inhibitor) increases incretin hormones, which raise insulin release and reduce glucagon after meals. Metformin reduces liver glucose production and improves insulin sensitivity. Pioglitazone (TZD) increases insulin sensitivity in muscle and adipose tissue, improving glucose uptake.
Oral tablet; take exactly as prescribed. Usually taken with meals to reduce stomach upset; swallow whole with water. Monitor blood glucose regularly; do not stop abruptly without medical advice.
Nausea, diarrhoea, abdominal discomfort, headache, dizziness, peripheral oedema (swelling), weight gain, upper respiratory tract infection
For type 2 diabetes only; not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, alcoholism, or hypoxia; assess renal function before and during treatment and stop temporarily before iodinated contrast/surgery as advised. Pioglitazone can cause fluid retention and may worsen heart failure; monitor for rapid weight gain/shortness of breath. Risk of hypoglycaemia increases when used with insulin or sulfonylureas. Sitagliptin has been associated with pancreatitis and severe joint pain; seek care for severe abdominal pain. Use caution in liver disease; monitor liver enzymes. Periodic vitamin B12 monitoring may be needed with long-term metformin.
Alcohol (increases lactic acidosis risk with metformin), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate/acetazolamide), cationic drugs eliminated by renal secretion (e.g., cimetidine), insulin and sulfonylureas (increased hypoglycaemia risk), corticosteroids/thyroid hormones/diuretics (may raise glucose), rifampicin (may reduce pioglitazone effect), gemfibrozil (may increase pioglitazone levels)
