Type 2 diabetes mellitus in adults, when diet and exercise alone or single-drug therapy is insufficient; helps control blood glucose levels
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin (a biguanide) decreases glucose production in the liver and improves insulin sensitivity, reducing insulin resistance. Together they improve fasting and post-meal blood sugar control.
Take by mouth as prescribed by your doctor, usually once daily with food (preferably with/after the main meal) to reduce stomach upset and risk of low sugar. Swallow the tablet whole; do not crush or chew (especially if sustained/extended release). Monitor blood glucose regularly.
Hypoglycaemia (low blood sugar), nausea, diarrhoea, abdominal discomfort, vomiting, headache, dizziness, taste disturbance, weight gain, skin rash
Risk of hypoglycaemia (higher with missed meals, strenuous exercise, alcohol, or with other antidiabetics); carry glucose/sugar source. Metformin can rarely cause lactic acidosis—seek urgent care for deep/rapid breathing, severe weakness, muscle pain, unusual sleepiness. Avoid/use with caution in kidney impairment, severe liver disease, severe infection, dehydration, heart/respiratory failure, or alcoholism. Temporarily stop metformin before/after iodinated contrast imaging or major surgery as advised. Use caution in elderly and those with renal impairment; periodic kidney function and vitamin B12 monitoring recommended.
Alcohol (increases risk of hypoglycaemia and lactic acidosis), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids/diuretics (may raise blood glucose), ACE inhibitors (may increase hypoglycaemia risk), rifampicin (may reduce glimepiride effect), azole antifungals/macrolides (may increase sulfonylurea effect), iodinated contrast media (lactic acidosis risk with metformin), cimetidine (may increase metformin levels)
