Prevention and treatment of acute gout attacks; prophylaxis of gout flares when starting urate-lowering therapy. Also used in some cases for familial Mediterranean fever as prescribed.
Colchicine reduces inflammation by inhibiting microtubule formation and neutrophil activity at sites of urate crystal deposition. This decreases inflammatory mediator release and pain/swelling during gout flares.
Take as directed by a doctor. Swallow tablet with water, with or without food (take with food if stomach upset occurs). For gout flare: dosing is doctor-specific; do not exceed prescribed dose. For prophylaxis: take at the same time daily.
Diarrhoea, nausea, vomiting, abdominal pain/cramps, loss of appetite
Use only on prescription; narrow therapeutic index. Do not use in severe kidney or liver impairment, or with strong CYP3A4/P-gp inhibitors due to risk of life-threatening toxicity. Stop and seek medical help if severe diarrhoea/vomiting, muscle pain/weakness, numbness/tingling, unusual bleeding/bruising, fever, or signs of infection occur. Use caution in elderly and in renal/hepatic disease; dose adjustment may be needed. Avoid grapefruit/grapefruit juice.
Strong CYP3A4/P-gp inhibitors (clarithromycin, erythromycin, ketoconazole/itraconazole, ritonavir/cobicistat, cyclosporine, verapamil/diltiazem) increase toxicity risk; statins/fibrates increase myopathy risk; digoxin may increase toxicity risk; other myelosuppressive drugs increase blood disorder risk; grapefruit juice increases levels
