How Topaz 100 mg Tablet works

Topiramate is an antiepileptic medicine that stabilizes overactive nerve activity in the brain. It works by modulating voltage-gated sodium channels, enhancing GABA activity, and reducing excitatory glutamate transmission, thereby lowering seizure risk and helping prevent migraines.

How to take Topaz 100 mg Tablet

Tablet for oral use. Take exactly as prescribed, at the same time each day, with or without food; swallow with water. Do not stop suddenly (may worsen seizures); dose is usually titrated gradually. Maintain adequate fluid intake to reduce kidney stone risk.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause deep unconsciousness, slowed breathing and dangerous heart rhythm disturbance. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Most side effects of Topaz are mild and settle as the body adjusts. Reported side effects include:

Warnings and precautions

Prescription-only antiepileptic. May cause drowsiness/dizziness—avoid driving or operating machinery until effects are known. Risk of metabolic acidosis, kidney stones, decreased sweating/heat intolerance (especially in children), and acute myopia/secondary angle-closure glaucoma—seek urgent care for eye pain/vision changes. Can cause mood/behavior changes and suicidal thoughts—monitor. Dose adjustment may be needed in kidney disease; caution in liver disease. Do not stop abruptly. May reduce effectiveness of hormonal contraception at higher doses—use additional contraception as advised.

Do not take Topaz if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Valproate-containing medicines are contraindicated in pregnancy for epilepsy unless no alternative exists, and in all women of childbearing potential without a pregnancy prevention programme. Several are not suitable in significant liver disease.

Interactions with other medicines

Oral contraceptives (reduced efficacy, especially ethinyl estradiol at higher doses), valproate (hyperammonemia/encephalopathy risk), other CNS depressants/alcohol (increased sedation), carbonic anhydrase inhibitors like acetazolamide (increased kidney stone/metabolic acidosis risk), phenytoin/carbamazepine (may alter topiramate levels), metformin (metabolic acidosis risk), lithium (level changes)
