Type 2 diabetes mellitus: to improve blood sugar control in adults when diet and exercise alone or single-drug therapy is inadequate
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin reduces glucose production by the liver and improves insulin sensitivity, lowering blood sugar levels.
Oral tablet. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew if it is SR/ER. Regularly monitor blood glucose; follow diet and exercise advice.

Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, heavy alcohol use, severe infection, liver disease, heart failure, and advanced age; kidney function should be assessed before and during treatment. Stop temporarily before/after iodinated contrast imaging or major surgery as advised. Monitor for pancreatitis symptoms (severe abdominal pain), hypersensitivity/skin reactions, and vitamin B12 deficiency with long-term use. Use caution in elderly and in patients with renal/hepatic impairment.
