Relief and prevention of bronchospasm in asthma, chronic bronchitis and COPD (chronic obstructive pulmonary disease); helps reduce wheezing, breathlessness and chest tightness
Theophylline is a methylxanthine bronchodilator. It relaxes airway smooth muscles and reduces airway responsiveness mainly by inhibiting phosphodiesterase and antagonising adenosine receptors, leading to bronchodilation and improved airflow.
Tablet (sustained release): take exactly as prescribed, usually after food with a glass of water. Swallow whole; do not crush/chew/break. Take at the same time each day; dose may be adjusted based on response and blood levels.
Nausea, vomiting, stomach upset/heartburn, headache, tremor, restlessness/insomnia, palpitations, increased urination
Narrow therapeutic index—overdose can cause severe vomiting, fast/irregular heartbeat, seizures. Use with caution in liver disease, heart disease/arrhythmias, hypertension, hyperthyroidism, peptic ulcer, epilepsy, and elderly. Smoking and fever/infections can change blood levels. Avoid excessive caffeine. Seek urgent care for severe palpitations, persistent vomiting, confusion or seizures.
Macrolide antibiotics (erythromycin/clarithromycin), fluoroquinolones (ciprofloxacin), cimetidine, oral contraceptives, fluvoxamine increase levels; rifampicin, phenytoin, carbamazepine, phenobarbital and smoking decrease levels; beta-agonists and caffeine increase side effects; lithium levels may decrease; alcohol may increase adverse effects
