Management of hypertension, angina pectoris (chest pain), chronic stable heart failure (as part of combination therapy), prevention of cardiovascular events in patients with cardiac risk as advised by physician
Metoprolol is a selective beta-1 adrenergic receptor blocker. It reduces heart rate and contractility, lowering cardiac workload and blood pressure. The XR formulation provides sustained drug release for 24-hour control.
Oral extended-release tablet; take exactly as prescribed, usually once daily at the same time each day. Swallow whole with water; do not crush or chew. Can be taken with or without food (taking with food may improve consistency). Do not stop suddenly without medical advice.
Dizziness, fatigue, slow heartbeat (bradycardia), low blood pressure, headache, cold hands/feet, nausea, sleep disturbances
Do not stop abruptly (risk of worsening angina, heart attack, arrhythmia). Use with caution in asthma/COPD (may cause bronchospasm), diabetes (may mask hypoglycemia symptoms), thyroid disorders (may mask hyperthyroid symptoms), peripheral vascular disease, and in patients with conduction disorders (AV block) or severe bradycardia. Monitor blood pressure and pulse. May cause dizziness—avoid driving if affected.
Other antihypertensives (additive hypotension), calcium channel blockers like verapamil/diltiazem (bradycardia/heart block), antiarrhythmics like amiodarone (conduction effects), digoxin (bradycardia), clonidine (rebound hypertension if stopped improperly), NSAIDs (may reduce antihypertensive effect), CYP2D6 inhibitors such as fluoxetine/paroxetine/quinidine (increase metoprolol levels), insulin/oral antidiabetics (mask hypoglycemia symptoms)
