Type 2 diabetes mellitus (to control high blood sugar), prediabetes (as advised), polycystic ovary syndrome (PCOS) with insulin resistance (off-label/when prescribed)
Metformin improves insulin sensitivity and decreases glucose production by the liver (hepatic gluconeogenesis). It also reduces intestinal absorption of glucose and improves peripheral uptake, helping lower blood sugar without usually causing hypoglycaemia when used alone.
Oral tablet. Take exactly as prescribed, usually with or after meals to reduce stomach upset. Swallow with water; do not crush unless advised. Dose is individualized; regular monitoring of blood glucose/HbA1c is recommended.
nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, flatulence, vitamin B12 deficiency (long term)
Contraindicated in severe renal impairment; assess kidney function before starting and periodically. Risk of lactic acidosis (rare but serious) increases with kidney/liver disease, dehydration, severe infection, alcohol misuse, heart/respiratory failure—seek urgent care for rapid breathing, severe weakness, abdominal pain, or unusual sleepiness. Temporarily stop around iodinated contrast studies and major surgery as advised. Use caution in elderly and in conditions causing hypoxia or dehydration.
Alcohol (increases lactic acidosis risk), iodinated contrast media, cimetidine, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), diuretics, corticosteroids, thyroid hormones, sympathomimetics, certain antipsychotics (may raise glucose), insulin/sulfonylureas (increased hypoglycaemia risk when combined)
Store below 25°C in a cool, dry place. Protect from moisture and direct light. Keep in original blister pack and out of reach of children.
