Type 2 diabetes mellitus (to improve glycaemic control) along with diet and exercise; may be used when metformin alone is inadequate
Dapagliflozin is an SGLT2 inhibitor that reduces glucose reabsorption in the kidneys, increasing urinary glucose excretion. Metformin decreases hepatic glucose production and improves insulin sensitivity, helping lower blood glucose.
Oral tablet. Take exactly as prescribed. Metformin-containing tablets are usually taken with meals to reduce stomach upset. Maintain adequate hydration; do not crush/chew unless advised.
Nausea, diarrhoea, abdominal discomfort, decreased appetite, headache, increased urination, genital fungal infection, urinary tract infection
Prescription medicine. Risk of lactic acidosis with metformin (higher risk in kidney impairment, dehydration, severe infection, alcohol misuse); kidney function should be assessed before and during therapy. Dapagliflozin may cause dehydration/low blood pressure and genital mycotic infections; rarely ketoacidosis can occur even with near-normal glucose—seek urgent care for nausea, vomiting, abdominal pain, rapid breathing. Temporarily stop during major surgery or severe illness and before iodinated contrast as advised by doctor. Use caution in elderly and those on diuretics. Not for type 1 diabetes.
Diuretics (increased dehydration/hypotension), insulin/sulfonylureas (increased hypoglycaemia risk), ACE inhibitors/ARBs (may increase risk of low BP with SGLT2), iodinated contrast media (increased lactic acidosis risk—metformin may need to be withheld), alcohol (increases lactic acidosis risk), cationic drugs affecting renal tubular secretion e.g., cimetidine (may increase metformin levels)
