Monograph
Tablet
Type 2 diabetes mellitus to improve blood glucose control along with diet and exercise; may be used when single-agent therapy is inadequate.
Dapagliflozin is an SGLT2 inhibitor that reduces glucose reabsorption in the kidneys, increasing urinary glucose excretion and lowering blood sugar. Metformin decreases hepatic glucose production and improves insulin sensitivity, reducing glucose absorption and improving peripheral uptake.
Oral tablet. Take as prescribed, usually once or twice daily with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Maintain adequate hydration and follow diet/exercise advice; monitor blood glucose regularly.
Nausea, diarrhoea, abdominal discomfort, decreased appetite, headache, increased urination, genital fungal infection, urinary tract infection, dizziness
For type 2 diabetes only (not for type 1 diabetes). Risk of lactic acidosis with metformin (higher in kidney/liver impairment, dehydration, severe infection, alcohol misuse); stop and seek care if unexplained weakness, muscle pain, fast breathing or severe drowsiness occur. Dapagliflozin may cause dehydration/low BP, genital infections, UTIs, and rarely ketoacidosis even with near-normal sugars; seek care for nausea/vomiting, abdominal pain, rapid breathing. Avoid/use with caution in moderate to severe renal impairment; kidney function should be checked before and during treatment. Temporarily withhold during acute illness, poor intake, major surgery, or before iodinated contrast procedures as advised by doctor.
Insulin and sulfonylureas (increased hypoglycaemia risk), diuretics (increased dehydration/hypotension), ACE inhibitors/ARBs (may increase risk of low BP), corticosteroids and sympathomimetics (may raise glucose), iodinated contrast media (increases lactic acidosis risk with metformin; temporary discontinuation advised), cationic drugs affecting renal tubular secretion (e.g., cimetidine) may increase metformin levels, alcohol (increases lactic acidosis risk)
