Treatment of angina pectoris (chronic stable/vasospastic), management of hypertension; also used in certain supraventricular arrhythmias such as atrial fibrillation/flutter for rate control (as advised by doctor)
Diltiazem is a calcium channel blocker that reduces calcium entry into heart and blood vessel smooth muscle cells. It relaxes blood vessels and decreases heart rate/contractility, reducing blood pressure and myocardial oxygen demand, helping prevent angina.
Take orally as prescribed, usually once or twice daily depending on the formulation. Swallow tablet whole with water; do not crush or chew sustained/controlled-release tablets. Can be taken with or without food, but take at the same time each day.
Headache, dizziness, flushing, ankle swelling (edema), constipation, nausea, fatigue, slow heart rate (bradycardia)
Use with caution in low blood pressure, heart failure, sick sinus syndrome, AV block, or slow heart rate unless paced; may worsen conduction abnormalities. Monitor blood pressure and pulse; do not stop suddenly without medical advice. Caution in liver or kidney impairment and in elderly. Seek care for fainting, severe dizziness, shortness of breath, worsening swelling, or very slow pulse.
Beta-blockers (e.g., metoprolol, propranolol) and digoxin (increased risk of bradycardia/AV block), ivabradine (avoid), amiodarone and other antiarrhythmics (additive conduction effects), statins metabolized by CYP3A4 (simvastatin/lovastatin—risk of myopathy; dose limits may apply), CYP3A4 inhibitors/inducers (e.g., clarithromycin/ketoconazole, rifampicin), cyclosporine/tacrolimus (increased levels), antihypertensives (additive BP lowering)
Store below 25°C in a dry place, protected from light and moisture. Keep in original blister pack and out of reach of children.
