Empagliflozin is an SGLT2 inhibitor that lowers blood sugar by increasing urinary glucose excretion through the kidneys. Metformin (ER) reduces glucose production in the liver and improves insulin sensitivity, helping the body use insulin better.
Oral extended-release tablet. Take exactly as prescribed, usually once daily with the evening meal or with food to reduce stomach upset. Swallow whole; do not crush, break, or chew. Maintain adequate hydration and follow diet/exercise advice; monitor blood glucose.

For Type 2 diabetes only (not for Type 1 diabetes or diabetic ketoacidosis). Risk of lactic acidosis with metformin—higher in severe kidney disease, dehydration, heavy alcohol use, liver disease, or hypoxia; stop and seek urgent care for rapid breathing, severe weakness, abdominal pain. Empagliflozin can cause dehydration/low BP, ketoacidosis (even with near-normal sugars), serious genital infections (Fournier’s gangrene), and increased risk of urinary/genital infections. Assess kidney function before and during therapy; avoid in severe renal impairment (eGFR thresholds per prescriber). Temporarily stop before major surgery or during acute illness with reduced intake/fluids. Use caution in elderly and those on diuretics. Consider vitamin B12 monitoring with long-term metformin.
Diuretics (increased dehydration/hypotension), insulin or sulfonylureas (increased hypoglycaemia risk), ACE inhibitors/ARBs (may enhance BP lowering), NSAIDs and iodinated contrast (may worsen kidney function and increase lactic acidosis risk), carbonic anhydrase inhibitors e.g., topiramate (metabolic acidosis risk), cimetidine (increases metformin levels), alcohol (increases lactic acidosis risk), digoxin (empagliflozin may increase levels slightly)
