Type 2 diabetes mellitus: as an adjunct to diet and exercise to improve glycaemic control; may be used when dual therapy is needed.
Dapagliflozin is an SGLT2 inhibitor that lowers blood glucose by increasing urinary glucose excretion from the kidneys. Metformin reduces hepatic glucose production and improves insulin sensitivity, thereby decreasing blood glucose levels.
Oral tablet. Take exactly as prescribed by the doctor. Usually taken once daily with food (often with the first/main meal) to reduce stomach upset; swallow whole if extended-release—do not crush or chew. Maintain adequate hydration and follow diet/exercise advice.

For type 2 diabetes only (not for type 1 diabetes/diabetic ketoacidosis). Risk of lactic acidosis with metformin (higher in kidney impairment, dehydration, severe infection, alcohol use); kidney function must be checked before and during therapy. Dapagliflozin can cause genital/urinary infections, dehydration/low BP, and rarely ketoacidosis even with near-normal sugars—seek care for nausea, vomiting, abdominal pain, rapid breathing. Temporarily stop during major surgery, severe illness, prolonged fasting, or dehydration; may need to stop before iodinated contrast procedures as advised. Use caution in elderly, those on diuretics, low BP, liver disease. Monitor blood glucose and HbA1c regularly.
Insulin and sulfonylureas (increased hypoglycaemia risk), diuretics (increased dehydration/low BP), ACE inhibitors/ARBs/other antihypertensives (additive hypotension), corticosteroids and sympathomimetics (raise glucose), iodinated contrast media (metformin—lactic acidosis risk; hold as advised), alcohol (increases lactic acidosis risk), cationic drugs cleared by kidneys such as cimetidine (may increase metformin levels)
