How It Works
Nifedipine is a dihydropyridine calcium channel blocker. It relaxes and widens blood vessels by reducing calcium entry into vascular smooth muscle, lowering blood pressure and improving blood flow to the heart, thereby reducing angina.
Directions for Use
Take by mouth as prescribed, usually once daily. Swallow the tablet whole with water; do not crush, chew, or split (retard/controlled-release). Can be taken with or without food; take at the same time each day.
Prescription medicine. Do not stop abruptly without medical advice. Use with caution in severe aortic stenosis, heart failure, low blood pressure, recent myocardial infarction, and liver impairment (may need dose adjustment). May cause dizziness—avoid driving/operating machinery until you know the effect. Seek urgent care for severe chest pain, fainting, marked swelling, or allergic reactions.
Drug Interactions
CYP3A4 inhibitors (e.g., erythromycin/clarithromycin, ketoconazole/itraconazole, ritonavir/cobicistat) may increase nifedipine levels; CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin) may reduce effect. Additive BP-lowering with other antihypertensives (beta-blockers, ACE inhibitors/ARBs, diuretics). Grapefruit juice increases levels; avoid. Caution with digoxin (levels may change) and magnesium sulfate (hypotension).
