Chronic heart failure (especially heart failure with reduced ejection fraction) to reduce risk of cardiovascular death and hospitalization
Sacubitril inhibits neprilysin, increasing beneficial natriuretic peptides that promote vasodilation and sodium excretion. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.
Take by mouth as prescribed, usually twice daily, with or without food. Swallow whole with water at the same times each day; dose titration is based on blood pressure, kidney function and tolerability. Do not stop abruptly without medical advice.
Low blood pressure (dizziness),increased potassium (hyperkalaemia),kidney function worsening (raised creatinine),cough,diarrhoea,fatigue,headache,angioedema (rare but serious)
Contraindicated with ACE inhibitors (require 36-hour washout when switching), history of angioedema related to ACEi/ARB, and pregnancy. Use with caution in low blood pressure, dehydration, kidney impairment, liver disease, and high potassium; monitor BP, serum potassium and renal function regularly. Seek urgent care if facial/lip/tongue swelling or breathing difficulty occurs.
