Type 2 diabetes mellitus: adjunct to diet and exercise to improve glycaemic control in adults when metformin/sitagliptin alone is inadequate or as replacement therapy.
Sitagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia alone.
Take as prescribed by physician. Usually taken with or after meals to reduce stomach upset. Place the MD tablet on the tongue to dissolve and swallow (with/without water). Do not crush/chew unless advised; follow dose adjustments based on kidney function and blood glucose.
Nausea, vomiting, diarrhoea, abdominal discomfort, loss of appetite, headache, dizziness, upper respiratory tract infection, hypoglycaemia (more likely with insulin/sulfonylureas), taste disturbance
Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, severe infection, alcohol misuse, heart failure, liver disease, or low oxygen states; stop and seek urgent care if deep/rapid breathing, severe weakness, abdominal pain occur. Use caution/avoid if eGFR is significantly reduced; monitor renal function periodically. Temporarily stop metformin around iodinated contrast studies or major surgery as advised. Sitagliptin has been associated rarely with pancreatitis and severe joint pain; stop and consult doctor if severe abdominal pain. Monitor for allergic reactions and bullous pemphigoid symptoms.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, diuretics, corticosteroids, thyroid hormones, sympathomimetics, antipsychotics (may raise glucose), ACE inhibitors (may enhance glucose-lowering), insulin/sulfonylureas (increased hypoglycaemia risk), digoxin (sitagliptin may increase levels)
