Monograph
Telmisartan (40mg) + Hydrochlorothiazide (12.5mg)
Treatment of hypertension (high blood pressure); helps reduce risk of cardiovascular events such as stroke/heart attack in high-risk patients as advised by doctor.
Telmisartan is an angiotensin II receptor blocker (ARB). It blocks the action of angiotensin II on AT1 receptors, causing relaxation of blood vessels and reduced aldosterone effects. This lowers blood pressure and decreases strain on the heart.
Take by mouth exactly as prescribed, usually once daily at the same time each day, with or without food. Swallow tablet whole with water; do not stop abruptly without medical advice. Dose adjustments may be needed based on BP response and kidney function.
Dizziness, headache, fatigue, nausea, back pain, increased potassium (hyperkalemia), low blood pressure (hypotension)
Not recommended in pregnancy (especially 2nd/3rd trimester) due to risk of fetal harm; stop and consult doctor immediately if pregnant. Use caution in breastfeeding, dehydration, low BP, kidney disease/renal artery stenosis, liver disease, and in elderly. Monitor blood pressure, kidney function (serum creatinine) and potassium, especially when starting or increasing dose. Risk of hypotension in volume-depleted patients (vomiting/diarrhea/diuretics). Avoid potassium supplements/salt substitutes unless prescribed.
Potassium supplements or potassium-sparing diuretics (spironolactone, eplerenone, amiloride) may increase hyperkalemia risk; NSAIDs (ibuprofen, diclofenac) may reduce effect and worsen kidney function; lithium may increase lithium levels/toxicity; other antihypertensives/diuretics may cause additive low BP; ACE inhibitors or aliskiren increase risk of kidney impairment/hyperkalemia (avoid aliskiren in diabetes).
Store at room temperature (below 25°C), protected from moisture and light. Keep in original blister pack and out of reach of children.
