Treatment of certain brain cancers such as glioblastoma multiforme (newly diagnosed and recurrent) and anaplastic astrocytoma, as prescribed by an oncologist
Temozolomide is an oral alkylating agent that converts to an active metabolite (MTIC) in the body. It damages tumor DNA (methylation), leading to inhibition of DNA replication and cancer cell death.
Take exactly as prescribed by the oncologist. Swallow capsule whole with water on an empty stomach (e.g., 1 hour before or 2 hours after food), preferably at the same time each day; do not open/crush/chew. If vomiting occurs after a dose, do not take an extra dose—take the next scheduled dose. Follow prescribed antiemetic and cycle schedule.
Nausea, vomiting, loss of appetite, constipation, fatigue, headache, hair loss, rash, low white blood cell count (neutropenia), low platelet count (thrombocytopenia)
Cytotoxic chemotherapy—use only under specialist supervision. Can cause severe myelosuppression; regular CBC monitoring required. Increased risk of infections and bleeding; seek care for fever, sore throat, unusual bruising/bleeding. Risk of hepatotoxicity—monitor liver function. Pneumocystis jirovecii pneumonia (PJP) prophylaxis may be needed (especially with concomitant radiotherapy). Avoid pregnancy; effective contraception required for men and women; may affect fertility. Do not handle broken capsules; avoid contact with powder.
Other myelosuppressive drugs (increased risk of cytopenias), valproic acid (may reduce temozolomide clearance/increase exposure), live vaccines (avoid due to immunosuppression), corticosteroids/radiotherapy (increased infection risk; PJP risk)
Store at room temperature (below 25°C), protected from light and moisture, in original pack. Keep out of reach of children.
