Treatment of hypothyroidism, thyroid hormone replacement after thyroidectomy or radioiodine therapy, TSH suppression in certain thyroid cancers/goitre (as prescribed)
Levothyroxine is a synthetic form of the thyroid hormone T4. It is converted to T3 in the body and restores normal thyroid hormone levels, helping regulate metabolism, energy, heart rate and body temperature.
Take orally once daily on an empty stomach, preferably in the morning 30–60 minutes before breakfast with water. Take at the same time each day; avoid taking with calcium/iron/antacids within 4 hours. Dose is individualized based on TSH/T4 and clinical response; do not stop or change dose without medical advice.

Use only under medical supervision; overtreatment can cause hyperthyroidism, angina, arrhythmias and bone loss/osteoporosis. Use caution in elderly and in patients with coronary artery disease, hypertension, diabetes or adrenal insufficiency (treat adrenal insufficiency first). Not for weight loss; high doses can be dangerous. Monitor TSH/T4 periodically, especially after dose changes and during pregnancy.
Reduced absorption with calcium salts,iron salts,aluminium/magnesium antacids,sucralfate,cholestyramine/colestipol,sevelamer; foods/soy/high-fibre can reduce absorption. Enzyme inducers (phenytoin,carbamazepine,rifampicin) may reduce effect. Potentiates anticoagulant effect of warfarin. May increase insulin/oral antidiabetic requirements; interacts with digoxin (dose adjustment may be needed).
